I have read, understand and accept the complete guidelines and regulations and agree to the following conditions if my project is selected:

1. Appropriate credit will be given to The Chicago Underground Film Fund on screen and in publicity. Appropriate credit is defined as acknowledgment equal to other acknowledgments for equal amounts from other grantors and sources. 

2. The project for which funds are sought will be in the final stages of completion at the time of award notification.

3. The film or tape will be made available to CUFF for a Chicago Area premiere screening.

4. Adequate accounting records will be made available to CUFF upon request.

5. When completed, you will provide a 1/2″ VHS cassette for CUFF archives.

6. Work will be completed within 12 months (one year) of receiving funds from CUFF.
